Makes rounds to evaluate status of residents and determine resource needs.
Meets with members of the interdisciplinary team to assess, plan, coordinate, and evaluate residents’ care.
Documents nursing activities accurately and in a timely manner.
Communicates staffing problems and needs to the Director of Nursing (DON).
Develops work assignment s for nursing assistants and other subordinate staff.
Reports problems and complaints to the Nursing Manager.
Provides feedback to nurse managers regarding performance of nursing assistants.
Receives and provides end of shift report.
Assists in admission, transfer, and discharge of residents.


Current TB test
Fingerprint Card
AZ State Nursing License
Good communication skills
Basic computer experience
Ability to organize and prioritize with good interpersonal skills
Previous long-term care experience a plus, but not necessary. We are willing to train the right entry-level applicants.


ADDITIONAL INFORMATION: Full-time employees start accruing Paid Time Off on the date of hire, Holiday Pay after a 90 day probation period, and are eligible for Healthcare Benefits the first of the month following 60 days of employment.

Oasis Pavilion Nursing and Rehabilitation Center is searching for (TWO) Physical Therapists for our Sub Acute Skilled Nursing Facility and Outpatient Rehabilitation Department. Oasis Pavilion is a 134 bed skilled nursing facility.

Candidate must have graduated from an APTA accredited school of Physical Therapy. Must possess a current Physical Therapy license in the State of Arizona, a current BLS certification, TB Screening and a level 1 fingerprint clearance card.

Job brief

We are looking for a qualified Dietary Aide to serve food to our patients in our facility. You will set up and deliver trays to patient rooms and ensure good sanitary conditions throughout the process. Set up the Dining room and serve appropriate meals to all patients that eat in the Dining Room.

A Dietary Aide must be capable to prepare all patient trays including desserts, drinks, and side items. Must follow tray card instructions while adhering to all safety and health standards. They must be competent and conscientious with a customer-oriented approach.

Our goal is to provide excellent food services to our patients and their family

– Set up tables or other dining areas with attention to cleanliness and order
– Deliver dishes and trays at designated times and rooms
– Takedown dining areas and collect plates, glasses, etc. 
– Discard leftovers and take out the garbage
– Clean food stations and wash dishes, cooking vessels, and silverware
– Stock ingredients and monitor inventory
– Assist in keeping track of needed supplies and condiments
– Help maintain kitchen equipment and appliances

Cook Job Summary:

The Lead AM cook works Monday – Friday, Weekends Off
The cook will prepare meals and follow establishment recipes. Duties include preparing ingredients,
adhering to the facility’s 4-week cycle menu. Able to follow food health and safety procedures.
Previous experience in a team environment and various cooking methods preferred. We are willing
to train for diet-restricted patients.

Cook Duties and Responsibilities:

– Setting up, cleaning, and organizing workstations
– Preparing ingredients for the shift; washing vegetables, chopping, seasoning meat, etc.
– Preparing oven, utensils, and grill for shift
– Cooking order according to food health and safety standards
– Ensuring each guest order has the correct food and sides
– Delivering food orders in a timely manner
– Storing all food properly
– Sanitizing and cleaning workstations and utensils
– Helping other team members when needed

Cook Requirements and Qualifications:

– Able to prioritize, and manage time efficiently
– Self-motivated and self-directed
– Works well as part of a team and on individual tasks
– Physically able to stand and move during the length of shift; able to bend and kneel through
shift; able to lift up to 25 pounds or more
– Must pass a background check
– Must be COVID vaccinated
